● Membership Fees are Non-Refundable.
● This membership fee recurs monthly unless canceled prior to the end of the term.
● All included services are rendered in-office and an appointment is required.
● We reserve the right to refuse membership to anyone at our discretion.
● We reserve the right to change the terms of this membership at any time without notice.
● Member discounts are not combinable with other promotional discounts.
● Reward points may not be combinable with member discounts.
● The membership is non-transferable.

CANCELLATION TERMS:
● A 6-month minimum commitment is required. Your membership will automatically renew month-to-month.
If you choose to cancel after six months, we require a 30-day written notice
● A cancellation fee of $50 will be charged if membership is canceled before the 6-month agreement
